15th may 2020: Therapeutic Pleural tap of 65yr old male patient with moderate left sided pleural effusion with CKD stage 3b.

16th may :Did 2d echo with unit chief for  pleural effusion with CKD case,to see whether there was a heart failure secondary to CKD. 

17th may : Sunday routine rounds of unit cases. 

18th may : studied pleural effusion, approach, diagnosis & treatment. 

19th may : OPD Duty day

20th may : Interpreted NCS report of our 22 yr old male with paraperesis. 

21st may : RCT role of statins for secondary prevention in pt's with ischemic stroke or TIA. 

22nd may : Assisted Endoscopic guided biopsy of 50 yr old male with ?cholangiocarcinoma

23rd may: Diabetic neuropathy & causes of small vs large fibre neuropathies. 

24th may: Sunday rounds of unit cases. 

25th may: casual leave

26th may: OPD duty day

27th may: Thesis data presentation & discussed about Acid base disturbances in diarrhea pt's

28th may: efficacy of Ivig & plasmapheresis in GBS. 

29th may: presented our suspecting GBS case to neurologist & took his opinion. 

30th may : Discussed with NCS technician about the method of NCS & Interpretation of NCS report of 18yr old male with ?GBS.

31st may: Did regular rounds of amc and ward patients.

Chief complaints:- A 24yr male who is a agricultural labourer came with complaints of Weakness of All limbs ( LL > UL ) , Urinary retention since 3 days. HOPI :- Patient is apparently asymptomatic 3 days back when he woke up at early morning 6am noticed weakness & Numbness in all limbs ( Lower limbs > upper limbs) due to which he is unable to roll or sit or get down the bed by himself so called his father for help. A patient by name ******,23 F, G1P1L1 delivered a female baby by vacuum assisted NVD on 10/7/2024 at private hospital, 20 minutes post delivery she had an Seizure ( GTCS type ) episode managed conservatively, Over next 2 hrs she had two more seizure episodes for which she is referred to Tertiary hospital for further management, There She had one more seizure episode & was unresponsive I/v/o low GCS patient is Intubated and Connected to MV. Patient is evaluated with CT brain which showed ill defined hypodensities along cortical & subcortical white matter of Right parietal lobe a possibility of PRESS to be considered. So Patient is referred to Our Hospital, Neurology Dept for Further management.
